Chronic Disease Management

If you have at least one chronic medical condition and complex care needs (as determined by your GP) you may be eligible for “UP TO five bulk billed visits” to an allied health practitioner under the Medicare Chronic Disease Management (CDM) scheme.

The SPEARS Health team of professionals works closely with you under this program to provide strategies and management plans that support the treatment and management of your condition. We also offer education to empower you, enhance your understanding, and promote a healthier lifestyle.

Cardiovascular Disease

Cardiovascular disease (CVD) is an umbrella term for a range of conditions that affect the heart and blood vessels. This includes heart disease, stroke, high blood pressure, heart failure, and conditions related to blood vessels such as atherosclerosis. CVD encompasses any disorder that impacts the heart’s ability to pump blood effectively or causes issues with the circulatory system.

Ensuring you are maintaining an active lifestyle and healthy diet are key to the prevention and early intervention of cardiovascular disease.

Diabetes

Whether this you have Type 1 or Type 2 Diabetes we are here to help you.

Our team of Exercise Physiologists, Physiotherapists and Dieticians will provide specialised support to controlling diabetes and enhancing overall well-being.

Falls Prevention 

One in three adults aged over 65 fall at least once per year. These incidents often lead to injury, disability and loss of independence.

Falls are a leading cause of injury-related hospitalisations among Australians aged 65 and over.. In 2021–22, this age group experienced a hospitalisation rate of 16,100 per 100,000 individuals. This rate increases with age.  

At SPEARS Health we acknnowlege falls can not only lead to injury, disabilty but loss of inndepedence and an increased anxiety. We will work with you to develop your strength, balance, hand-eye coordination and improved walking mechanics, to reduce your risk of falls. 

Neurological Conditions

Our Health Care Professionals can conduct a full physical assessment of your condition, monitor your condition over time, liaise with your GP and wider Multi Disciplinary Team to facilitate patient centred effect care. 

Physiotherapy plays a crucial role in the rehabilitation of patients with neurological conditions, such as stroke, Parkinson’s disease, multiple sclerosis, and spinal cord injuries, in an outpatient setting.
